See what needs attention at the shift change

A useful handoff explains what changed, what remains open, and where to start. Keep background in the appropriate record and use shift notes and tasks for the current priorities. Agree on what staff record at the end of each shift so the next person knows what to expect.
Walk through the shift together
- Check beds and arrivals. Review capacity, cleaning needs, and expected arrivals.
- Review the nightly count. Confirm census status and anything still needing clarification.
- Review open work. Make the next action and responsible staff member clear in your team’s routine.
- Ask for acknowledgment. Confirm important points directly with incoming staff using your facility’s process.
A saved note is not proof that someone has read it. Time-sensitive concerns still need your facility’s established communication and escalation process.
Write a next action someone can use
A fictional handoff: “Room 2’s empty bed is still marked cleaning. Jordan will check it at 9 p.m. and update the bed status before the next placement.” The incoming shift has a current status, a named owner, and a time to follow up.
Keep sensitive background out of general summaries. Share only what staff need for the work, using your approved access-controlled records.
